Silicon Valley Adult Day Health Care is Hiring a Social Worker, MSW (Bilingual in Mandarin) Near Milpitas, CA
Silicon Valley Adult Day Health Care Center, SV ADHC, a Community Based Adult Service (CBAS) facility, has a full time opening for MSW.
Duties: Participant assessment and care plan development, new participant Intake, home evaluation, discharge planning, individual/group support, and participant education.
Requirements:
Applicants need to satisfied "ONE of The Following Qualifications":
- Has a master's degree in social worker from an accredited school.
- Has a master's degree in psychology, gerontology or counseling from an accredited school and has one year of experience providing social services in the fields of aging, health or long-term care services.
- Has a license by the California Board of Behavioral sciences.
- Has a bachelor's degree in social work from an accredited school with two years experience providing social services in one or more of the field of aging, health or long-term care.
- Bilingual in Mandarin is a plus.
Job Type: Full-time
Pay: From $32.00 per hour
